Recently a teacher emailed me trying to find suggestions on how to create a teacher transcript that her students can complete anonymously. One of the things that I recommended was to create a Google Form that doesnt need students to sign into their Google accounts. Google Forms includes a course assessment design template. I would just use that template and change a couple of items within it to satisfy my requirements. Before sending out the Form to my trainees I d disable all of the alternatives for gathering e-mail addresses and any other recognizing information.
